March 19, 2014 Health Care Marketplace Help

(Dodgeville) Need help applying online to the new Health Insurance Marketplace? Free Dodgeville Help Center open on every Wednesday until March 26th from 2-6pm at the SW Tech Center (next to Piggly Wiggly Mall at 316 W. Spring St, Dodgeville).

Also, a Certified Application Counselor will be at the Community Connections Free Clinic on Tuesdays and Thursday from 2-5pm until March 27th.

Please bring along the following information for all family members and dependents to be covered on your insurance: 1) social security numbers and 2) the family's combined estimated 2014 income (W-2s, tax returns).
